@This system will be applied to the family that lives in this city and have a child under 12 to decrease the economical burden of the medical expense. This system is aiming for maintaining and promoting childfs well being. The municipal government will pay personal expense for medical fees of a child. However, those things are not covered with this financial support; meals when the child is in the hospitals, and what is not covered by the national health insurance (e.g. check-ups, medicine container, and so on).
How to apply:
You can apply to this at child section counter in the city hall. Bring national health insurance certificate, a personal seal. You must be applied to national health insurance. Fill the application forms given at the counter.
How to use this support at the medical facilities:
Show the proof of this financial support given after you are qualified at the medical facility or pharmacy. The expense not covered with national health insurance is paid by the Soja city directly to those medical facilities.
Things necessary for application:
Parents or guardians personal seal
National health insurance card which child names are listed on.
Reminder:
This financial support system is not valid at medical facilities outside of Okayama prefecture. If you paid your medical bills because you didnft have the proof, or you went to medical facilities outside of this prefecture, it would be refundable. Get the proof where you get medical service.
