| Application for replacement of the alien registration card |
Application for replacement of the alien registration card is required in the following cases
(1) The registration card is greatly damaged or stained.
(2) When a correction or change of the registry is made as to name, birthday, gender or nationality.
(3) When there is no space left to add a postscript on the entry area on the back of the registration card in changing or correcting the registry.
Document necessary for application:
Alien registration card
2 copies of photograph (45mm in length x 35mm in width taken within 6 months) Those 15 years of age or younger are not required.
Passport (if you have)
Reminder:
If your age is 15 or younger, a parent (or a relative living in the same residence if you do not have a parent) has to apply.
